Job Title: Street Medicine Program Manager

The Street Medicine Program Manager is responsible for overseeing the strategic direction, operational management, and quality improvement of the Street Medicine Program at One Community Health.

Key Responsibilities:
  • Develop and implement strategies to enhance program effectiveness and efficiency.
  • Ensure program goals and objectives are met and aligned with organizational priorities, mission, and values.
  • Manage day-to-day operations, including scheduling, logistics, and resource allocation.
  • Monitor and report on program performance metrics and outcomes.
  • Develop and maintain strong partnerships with community organizations, healthcare providers, and stakeholders.
  • Advocate for the needs of the unhoused population and represent the program in community forums and meetings.
  • Collaborate with partners to develop and implement outreach and engagement strategies.
  • Provide training, support, and professional development opportunities to ensure street medicine staff maximize efficiency and create a positive team environment.
  • Work with leadership to develop and manage the street medicine program budget, ensuring efficient use of resources and financial sustainability.
  • Implement and oversee quality improvement initiatives, including clinical outcomes.
  • Utilize data to assist in program evaluations, monitor program performance, and drive improvements when necessary.
  • Ensure patient satisfaction and address any concerns or complaints.
  • Support outreach to unhoused encampments to ensure individuals experiencing homelessness are aware of services.
Requirements:
  • Bachelor's Degree in Public Health, Healthcare Administration, Social Work, or a related field (Master's Degree preferred).
  • Minimum of 5 years of experience in program management, including strategic planning, budgeting, and staff supervision.
  • Strong leadership and interpersonal skills.
  • Ability to work effectively in a diverse, multidisciplinary team environment.
  • Excellent communication and advocacy skills.
  • Commitment to serving vulnerable populations and addressing health disparities.
Preferred Background:

Individuals with life experience in these areas contribute to our ability to serve this population more effectively.

Demonstrated person-centered approach and familiarity with trauma-informed systems and restorative practices.

Experience in street medicine, homeless healthcare, or related fields.

Additional training or certification in public health or healthcare administration.

Bilingual skills.
