We are looking for a Board Eligible/Board Certified physician with fellowship training in Geriatric Medicine (Internal Medicine or Family Practice) to join our accomplished team of geriatricians and advanced practice clinicians.
The Geriatrics Division emphasizes coordinated care across various settings, employing a collaborative approach to meet the needs of our patients. Opportunities exist to engage with trainees from prestigious institutions and to participate in innovative research initiatives focused on chronic disease management and telemedicine.

- Provide expert consultation at the Memory Care Clinic and Geriatric Clinic, ensuring ample time for patient interactions
- Deliver comprehensive medical services in post-acute and long-term care environments, particularly in Skilled Nursing Facilities
- Conduct detailed assessments and create individualized care plans for geriatric patients
- Collaborate with interdisciplinary teams to enhance care coordination and efficiency
- Engage in patient education and shared decision-making to empower older adults
- Work alongside specialists for complex patient cases
Qualifications:
- Active medical license in California and DEA registration
- Board Eligible/Board Certified in Geriatric Medicine (Internal Medicine or Family Practice)
- Completion of an ACGME accredited fellowship in Geriatric Medicine
- Strong clinical expertise in geriatric care
- Excellent communication and interpersonal skills
- Ability to thrive in a collaborative healthcare environment

Compensation Structure:
$131.65 per hour
