Job SummaryThe Licensed Clinical Social Worker will play a critical role in our team, providing initial evaluations and individual and group therapy to patients. This is an exciting opportunity to join a dynamic and growing organization that is committed to innovation and excellence in healthcare.
Key Responsibilities:- Conduct initial evaluations and assessments of patients' mental status and develop individualized treatment plans.
- Provide individual, family, and group therapies to patients, utilizing evidence-based practices and techniques.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care and coordination.
- Develop and maintain relationships with community-based resources and services to support patients' social determinants of health.
- Ensure accurate and timely documentation of patient progress and treatment plans.
- Master's degree in social work from an accredited program.
- Current licensure as a Licensed Clinical Social Worker with at least three years of behavioral healthcare experience.
- Experience working in a primary healthcare setting, preferably with a geriatric population.
- Bilingual in Spanish is a plus.
