Treatment Coordinator Job Description
We are seeking a compassionate and experienced Treatment Coordinator to join our team at PRC Baker Places. As a Treatment Coordinator, you will play a vital role in supporting the delivery of high-quality services to our clients and ensuring their overall well-being.

Our mission is to empower individuals affected by HIV/AIDS, substance use, or mental health issues by providing integrated legal, social, and health services that address their unique needs.

Key Responsibilities:
  • Assess client needs and develop individualized treatment plans.
  • Coordinate referrals to community resources and providers.
  • Monitor client progress and adjust treatment plans as needed.
  • Communicate effectively with clients, families, and healthcare providers.
  • Maintain accurate records and reports to meet regulatory requirements.

Requirements:
  • Bachelor's degree in Psychology, Social Work, or related field; Master's degree preferred.
  • At least one year of experience in a mental health or substance abuse treatment setting.
  • Cultural competency working with diverse populations on mental health, substance abuse, and HIV/AIDS issues.
  • Excellent communication and organizational skills.

Estimated Salary:$65,000 - $90,000 per year based on qualifications and experience.
