Position Overview

We are seeking a skilled RN or LPN to fulfill the role of MDS Coordinator. This position is crucial in overseeing, preparing, and managing the MDS process, which includes developing tailored care plans from the point of admission through to discharge. The MDS Coordinator is responsible for directing and allocating essential resources to uphold the company’s commitment to ensuring the highest possible quality of life and health for all residents.

Key Qualifications:

  • Proven experience in a healthcare environment that provides skilled nursing care, rehabilitation services, assisted living, or residential care.
  • Expertise in formulating individualized care plans.
  • Familiarity with standard assessment tools for evaluating resident conditions, including the RAI Manual.
  • Proficient in the MDS 3.0 system.
  • In-depth understanding of CMS payment and reimbursement policies.
  • Exceptional observational skills and the ability to critically assess extensive information.
  • Strong organizational skills that facilitate the management of multiple priorities and adherence to an effective schedule.
  • Valid RN or LPN license is mandatory.
