With a focus on patient-centered care, you'll work collaboratively with our team to ensure that every patient receives the best possible care.

Key Responsibilities:
  • Ensure that all activities align with Elara Caring's vision and values
  • Prevent unwanted hospitalizations by meeting patient and family needs in the home
  • Prepare and maintain accurate patient records, charts, and documents
  • Comply with applicable laws and regulations
  • Provide training and continuing education for staff
  • Assist in development of clinical practice guidelines/standards
  • Collaborate with Medical Director, attending physician, branch leadership, and IDG
Requirements:
  • Masters Degree in Nursing and certification through a national Nurse Practitioner Licensing board
  • 3+ years clinical nursing experience
  • 1+ year post-graduate clinical nursing experience in the hospice and palliative care field
  • Prior experience in Family Medicine, Geriatrics, and psychiatry preferred
  • Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations
  • Board Certification in Palliative Care Preferred
  • Active DEA license
  • Able and willing to travel up to 50% as needed
