Job Summary

Key Responsibilities
  • Coordinate care for patients across the healthcare continuum, ensuring timely and safe transitions.
  • Develop and implement effective discharge plans, working closely with internal and external resources.
  • Collaborate with patients, caregivers, and healthcare providers to plan and execute safe discharges.
  • Identify and facilitate post-acute resource needs, including home care, community-based referrals, and diagnostic testing.
  • Ensure that patient vulnerability has been captured and documented on the Transitions of Care report.
  • Review predictive tools for readmission and implement additional interventions to mitigate risk.
Requirements
  • Bachelor's in Science of Nursing
  • Registered Professional Nurse (RN) License in state of Illinois upon hire
  • Basic computer knowledge using word processing, spreadsheet, email, and web browser
