Position: Home Caregiver

At WelbeHealth, we embrace a unique approach to caregiving, striving to enhance the lives of our participants. Our Home Caregivers play an essential role in delivering support within the comfort of our participants' homes, while also actively engaging in the collaborative care planning process as integral members of our interdisciplinary team (IDT).

Reporting Structure: The Home Caregiver will be accountable to the Home Care Supervisor, ensuring the safe provision of personal care services to participants as per the established schedule and directives.

Key Responsibilities:

  • Deliver hands-on personal care and assist with daily living activities (ADLs) for participants, which includes bathing, grooming, dressing, toileting, and safe physical transfers.
  • Support participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Accurately measure and document vital signs as per instructions.
  • Assist with light meal preparation and feeding, ensuring participants maintain proper nutrition and hydration.
  • Facilitate safe transfers in and out of the home and serve as a medical escort for appointments as needed.
  • Contribute to the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following appropriate training.
  • Provide translation services when qualified and necessary.

The salary range for this position is $22.05 per hour, plus bonuses and equity. WelbeHealth provides a competitive total rewards package, including 401k matching, healthcare coverage, and a variety of other benefits. Actual compensation will be adjusted based on experience and qualifications.
