Position: Home Caregiver
At WelbeHealth, we embrace a unique approach to caregiving. Our Home Caregivers play an essential role in enhancing the lives of our participants by delivering compassionate support in their residences. As integral members of our interdisciplinary team (IDT), they contribute significantly to the care planning process, ensuring that our participants receive the highest quality of care.
Reporting Structure: The Home Caregiver will report directly to the Home Care Supervisor and is responsible for the safe provision of personal care services to participants in their homes, as per the established schedule.
Key Responsibilities:
- Deliver hands-on personal care and assist with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and transferring participants safely.
- Support participants with Instrumental Activities of Daily Living (IADLs), including light housekeeping, laundry, and shopping.
- Monitor and document vital signs as directed.
- Assist with meal preparation and feeding, ensuring that participants maintain proper nutrition and hydration.
- Facilitate safe transfers in and out of homes and provide medical escort services as required.
- Collaborate with the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following appropriate training.
- Offer translation services when qualified.
