Job Title: Nurse Practitioner/Physician Associate

Reports to: Medical Director of Iris Health Medical Group

Job Description:

The Nurse Practitioner/Physician Associate at Iris Health Medical Group provides high-quality medical care to patients, working under the general supervision of the Medical Director. This role requires clinical excellence, strong organizational skills, and exceptional interpersonal and communication skills. The ideal candidate will have a strong desire to work as a member of the care team to meet the challenges of building a novel care model for vulnerable patient populations.

Responsibilities:

  • Provide primary care services to medically and socially complex, elderly, and homebound patients.
  • Practice within the scope of the California Board of Registered Nursing and/or Physician Associate practices, under the oversight of the Medical Director, and within organizational guidelines.
  • Coordinate assessment, treatment, and management of patients through consultation with the supervising physician to facilitate comprehensive patient care plans.
  • Diagnose or treat acute healthcare problems, illnesses, infections, and injuries.
  • Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Provide health care screenings for both preventative health and chronic conditions.
  • Counsel and educate patients regarding preventative health, chronic conditions, and disease prevention.
  • Prescribe appropriate medications (dosage and route) utilizing best practices.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and healthcare needs.
  • Document in E.H.R. physical findings, formulate plan/prognosis based on patients' condition, and follow-up with patients in an accurate, timely manner.
  • Comply with all state and federal laws and regulations, as they pertain to the position, including HIPAA, sexual harassment, scope of practice, OSHA, etc.

Skills and Abilities:

The ideal candidate will possess excellent computer skills, preferably with Windows, including Microsoft Office Suite, and excellent telephone and communication skills. They will also have excellent organizational, writing, and verbal skills, with the ability to work independently, set priorities, and work well under pressure. Additionally, they will be able to maintain a high degree of confidentiality and demonstrate superior professionalism when dealing with patients and colleagues.

Minimum Qualifications:

  • Completion of a certified Family Practice Nurse Practitioner or Physician's Assistant program.
  • Current RN/NP or PA license with corresponding State Board of California.
  • Current active DEA registration or the ability to obtain upon hire.
  • Must successfully complete the provider credentialing process in accordance with National Accreditation requirements.
  • Current Basic Life Support certification required.
  • Experience as a Family Nurse Practitioner or Physician's Associate in the area of primary care, or experience in assessing, diagnosing, and treating a broad range of conditions, including medication management, under supervision of a Provider.
  • Care and compassion in working with low-income, indigent, and other racially, culturally, ethnically, and financially diverse populations.
  • Ability to chart visits comprehensively and complete E&M coding in compliance with medical coding standards.
  • Experience with eClinicalWorks or other electronic health record systems.
