Job Summary

The Patient Care Manager is a registered nurse who oversees the development and management of patient care outcomes. This role requires expertise in assessment, treatment, and care for patients of all ages, including neonate, pediatric, adolescent, adult, and geriatric populations.

This position involves complex discharge planning and utilization review. The ideal candidate will have 3 years of acute care hospital experience and a Bachelor's Degree in Nursing.

A minimum of 1 year of experience in home health, mental health, substance use, hospice, and/or utilization review is preferred. Progressive nursing leadership experience, including charge nurse roles, is also desired.
