Job Summary:
The Client Care Manager at True North Home Care is responsible for overseeing the quality care of our clients. This includes answering phone calls in a timely manner, responding to messages, and handling call-ins by finding suitable replacements for shifts. The Client Care Manager also helps keep care plans up to date and facilitates the training of team members.
Key Responsibilities:
- Answer the On-Call phone in a prompt manner and return calls within 10 minutes if missed.
- Problem solve, trouble-shoot, and triage clients and caregiver needs to ensure all scheduled clients are safe and cared for.
- Assist with other work necessary or important to the operation of the Company, including caregiver duties.
- Provide professional communication with clients, team members, and community members.
- Take new client intake calls and messages for non-urgent issues.
- Provide emotional support and compassionate care to clients and their families.
- Maintain required competency and skills.
- Participate in annual performance reviews.
- Report any changes in client condition or family situation immediately.
Requirements:
- Use of the AxisCare app for clocking in, clocking out, and documenting visit notes.
- Use of a mobile phone to respond to supervisor calls and texts during business hours.
- Adherence to all policies and procedure guidelines as well as OSHA guidelines.
- Attendance at required trainings and meetings.
- Maintenance of current certification as required by the State.
- Maintenance of current knowledge and practice of infectious disease protocols.
