Pacific Clinics is seeking a highly skilled and compassionate Housing Navigator to join our team. As a Housing Navigator, you will play a critical role in ensuring that individuals and families in need of housing support receive timely and quality services.
Key Responsibilities- Develop and implement housing support plans for clients, including identifying and securing available resources to assist with subsidizing rent and identifying affordable housing options.
- Collaborate with local Continuum of Care (CoC) homeless services systems, Managed Care Plans (MCPs), and Enhanced Care Management (ECM) providers to provide quality services to clients.
- Conduct pre-inspections of identified units to ensure they meet Housing Quality Standards (HQS) and assist clients with requesting security deposit assistance and setting up fees.
- Provide landlord/tenant mediation services to address issues and concerns, and help clients maintain their permanent housing.
- Maintain documentation of services provided as required by the program contract and Agency standards.
- Bachelor's degree or equivalent experience in a social service-related field or years of combined experience, related to case management, housing supportive services, and working with the homeless population.
- Knowledge of housing rights pertaining to homelessness, and low- and moderate-income housing.
- Valid California driver's license and maintain an insurable driving record under the agency's liability policy (if driving two or more times per week on company business) OR if driving is not required, demonstrated ability to use public transportation.
- Bachelor's degree or equivalent experience in a social service-related field.
- Knowledge of Coordinated Entry Systems (CES), local Continuum of Care system, local housing authorities, and ability to create partnerships with local landlords, realtors, property management companies as an effort to build housing stock.
