Job Summary and Qualifications

RN Clinical Transition Discharge Lounge Part-Time Days

As a Registered Nurse specializing in Admission and Discharge, your primary focus will be to deliver outstanding patient-centered care. You will be dedicated to ensuring a smooth and efficient transition for patients entering and exiting acute care hospital environments. Your specialized assessment skills will help guarantee that patients are placed in the appropriate care areas and discharged effectively. Education and post-hospitalization care will be provided as necessary. Your main objective will be to advocate for patients, families, and caregivers while embodying the organization's vision, mission, and values. The aim is to provide an unparalleled patient experience and achieve optimal clinical outcomes.

Key Responsibilities:

  • Conduct nursing assessments and ensure proper admission to inpatient care units. Take appropriate actions when necessary, such as communicating special circumstances to physicians or care teams, or reporting signs of abuse or neglect.
  • Complete and facilitate the necessary documentation, including admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
  • Educate patients, families, and caregivers about medical conditions, treatment plans, medications, potential side effects, and follow-up measures. Simplify complex medical terminology to ensure comprehensive understanding.
  • Complete additional discharge documentation, education, and care for specific patient populations to ensure compliance with discharge core measures.
  • Ensure that patients, families, and caregivers receive copies of discharge paperwork and any other necessary documentation, such as referrals for ongoing care and educational materials.
  • Collaborate with House Supervisors and Nursing Administration to prioritize workload and identify areas of greatest need to facilitate smooth patient transitions.
  • Demonstrate effective time management and the ability to prioritize tasks, distinguishing between low and high-priority activities.
Required Qualifications:
  • Basic Cardiac Life Support certification must be obtained within 30 days of employment start date.
  • Registered Nurse license from Texas State or Compact State is required.
  • Registered Nurse Diploma, Associate Degree, or Bachelor's Degree is necessary.
  • 1-2 years of experience in Admission Discharge Lounge or Medical Surgical/Tele environments.
