Job Summary:

We are seeking a compassionate and skilled Nurse Care Specialist to provide high-quality care to our senior adult patients in the comfort of their own homes. As a Private Duty Nurse LPN, you will play a vital role in assisting our patients with transitioning home from the hospital and providing the continuous care they need to remain at home with their loved ones.

Responsibilities:
  • Implement physician's prescribed plan of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in system of record
  • Provide health promotion, teaching, and training to family members
  • Accept and write verbal orders from physicians as required

Requirements:
  • 6 months prior hands-on nursing experience preferred but not required
  • Ability to work flexible schedules, including nights and weekends
  • Strong communication and interpersonal skills
