Clinical Pharmacist PNE-Central Operations, Casual Rotating

We are seeking a highly skilled Clinical Pharmacist to join our team at Northwestern Memorial Healthcare. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications across our healthcare system.
Key Responsibilities- Interpret medication orders to ensure safety and proper use of medications, identifying potential interactions, allergies, and administration errors.
- Coordinate with pharmacy, nursing, and prescribers to resolve medication-related problems, demonstrating initiative and sound judgment.
- Monitor medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions, intervening when necessary.
- Evaluate and document the appropriateness of medication selections for achieving desired therapeutic outcomes.
- Communicate potential problems and solutions to prescribers, acting as a consultant in pharmacokinetics and adverse reactions.
- Participate in therapeutic drug monitoring and intervention programs, adjusting therapy as indicated by patient condition and laboratory results.
- Process medication order information into the Electronic Health Record to ensure accuracy of records.
- Maintain comprehensive pharmacy patient profiles, including age, maturation, weight, allergies, diagnosis, and pharmacokinetic parameters.
- Oversee the packaging, labeling, and distribution of medications, ensuring compliance with established policies and guidelines.
- Monitor the use of scheduled medications on nursing units and maintain controlled inventories of medication in dispensing areas.
- Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
- Verify medication orders before delivery to patient care areas, resolving discrepancies as necessary.
- Review laboratory data for appropriateness of medication use, recommending changes when needed.
- Develop and present education and training programs for medical, nursing, and pharmacy staff, as well as assist in educating pharmacy students, residents, and fellows.
- Educate patients and their families about specific medications.
- Respond to emergency situations to assist with medication management.
- Provide accurate and timely medication information and teaching to healthcare professionals and patients.
- Maintain the hospital formulary and limit medication usage to formulary items when appropriate.
- Document all clinical activities and interventions performed.
- Collect data, maintain logs, records, and other documentation as required by the department.
- Communicate professional knowledge effectively in both oral and written forms to patients and other health professionals.
- Participate in quality improvement and medication use review activities of the department.
- Organize, prioritize, and plan workload within given time constraints.
- Work cooperatively with fellow pharmacists to oversee and direct the activities of the work area, assign work to technicians, and assure that established department objectives are accomplished.
- Review restocking automated dispensing cabinets throughout the hospital and resolve any problems.
- Ensure compliance with hazardous waste collection and disposal procedures.
- Maintain proficiency and expertise in areas of practice through continuous professional development.
- Current license to practice pharmacy in the State of Illinois.
- Bachelor's Degree in Pharmacy or a Doctor of Pharmacy Degree.
- Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.).
- Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
