Responsibilities:
  • Interpret medication orders to ensure safety and proper use of medications, identifying potential interactions and allergies.
  • Coordinate with pharmacy, nursing, and prescribers to resolve medication-related problems, demonstrating initiative and sound judgment.
  • Monitor medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions, intervening when necessary.
  • Evaluate and document the appropriateness of medication selections, communicating potential problems and solutions to prescribers.
  • Act as a consultant in pharmacokinetics and adverse reactions, providing expertise to physicians, nurses, and other healthcare professionals.
  • Participate in therapeutic drug monitoring and intervention programs, adjusting therapy as indicated by patient condition and laboratory results.
  • Process medication order information into the Electronic Health Record, ensuring accuracy and completeness.
  • Maintain comprehensive pharmacy patient profiles, including age, maturation, weight, allergies, diagnosis, and pharmacokinetic parameters.
  • Oversee the packaging, labeling, and distribution of medications, ensuring compliance with established policies and guidelines.
  • Monitor the use of scheduled medications on nursing units and maintain controlled inventories of medication in dispensing areas.
  • Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
  • Verify medication orders before delivery to patient care areas, ensuring accuracy and completeness.
  • Resolve discrepancies and review laboratory data for appropriateness of medication use, recommending changes when necessary.
  • Develop and present orientation, education, and training programs for medical, nursing, hospital, and pharmacy staff.
  • Educate patients and their families about specific medications, responding to emergency situations as needed.
  • Provide accurate and timely medication information and teaching to healthcare professionals and patients.
  • Maintain the hospital formulary and limit medication usage to formulary items when appropriate.
  • Document all clinical activities and interventions performed, collecting data and maintaining logs as required.
  • Communicate professional knowledge effectively in both oral and written forms to patients and other health professionals.
  • Participate in quality improvement and medication use review activities of the department.
  • Organize, prioritize, and plan workload within given time constraints, working cooperatively with fellow pharmacists to oversee and direct the activities of the work area.
Qualifications:
  • Current license to practice pharmacy in the State of Illinois.
  • Bachelor's degree in Pharmacy or a Doctor of Pharmacy Degree.
  • Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.) preferred.
  • Completion of a pharmacy residency program or equivalent hospital pharmacy experience preferred.
