About the Role

The Health Homes Care Coordinator is a critical role in our organization, responsible for working with enrolled members to develop and implement Health Action Plans (HAPs) that improve their quality of life, health, and engagement.

Key Responsibilities
  • Engage prospective Health Homes participants through referrals and outreach activities
  • Manage a caseload of 50 members, prioritizing their needs and coordinating care activities
  • Develop and implement HAPs, working with members to achieve their short-term and long-term goals
  • Provide follow-up services, including telephonic and face-to-face engagement, to ensure members receive necessary support and resources
  • Coordinate community outreach services, including home visits and transportation assistance
  • Track meetings with members and provide coaching to help them meet their goals
Requirements
  • Current license as a registered nurse, practical nurse, mental health counselor, or related field
  • Master's or bachelor's degree in social work, psychology, social services, human services, or behavioral sciences
  • At least two years of experience in social services, healthcare, or case management, preferably with fieldwork
  • Ability to travel to meet members, periodically for trainings and meetings
  • Excellent organizational and planning skills, with ability to work independently and prioritize productivity standards
