POSITION TITLE: Registered Nurse

PRIMARY DUTIES:

Provide comprehensive nursing care in alignment with the patient's care plan, which includes thorough health assessments, monitoring patient conditions, promoting health, coordinating services, and conducting educational activities.

KEY RESPONSIBILITIES:

  1. Oversee total patient care through comprehensive evaluations and continuous monitoring of health status.
  2. Assess the effectiveness of nursing services provided to patients and their families regularly.
  3. Handle all necessary documentation for patient admissions, re-assessments, transfers, and discharges.
  4. Present patient records to the Clinical Record Review Committee as required.
  5. Consult with authorized practitioners regarding modifications to Patient Care Plans.
  6. Coordinate patient services effectively.
  7. Submit clinical and progress notes consistently, detailing services rendered.
  8. Participate in case conferences to address patient-related issues with supervisors.
  9. Identify the need for additional health team members' involvement.
  10. Obtain orders for paraprofessional services and refer to appropriate personnel.
  11. Engage in discharge planning for patients.
  12. Collaborate with other agencies to ensure continuity of care.
  13. Participate in staff development meetings and ongoing education to enhance nursing skills.
  14. Contribute to the development and revision of the authorized practitioner's Plan of Care.
  15. Maintain up-to-date knowledge of current drug therapies.
  16. Adhere to the Washington nurse practice act.
  17. May be requested to assist other nursing staff as needed.

ADMISSION COORDINATION:

  1. Conduct initial and ongoing assessments of patient needs.
  2. Gather medical histories relevant to current conditions.
  3. Perform physical examinations, including vital signs and mental status evaluations.
  4. Evaluate home environments to determine available support for patient care.
  5. Explain nursing and agency services to patients and families.
  6. Develop and implement individualized nursing care plans.

SKILLED NURSING CARE:

  1. Deliver nursing services and treatments requiring specialized skills as ordered.
  2. Initiate preventive and rehabilitative nursing procedures as necessary.
  3. Monitor and report any changes in patient conditions to authorized practitioners.
  4. Educate and support patients and caregivers regarding care needs.

SUPERVISION OF HOME HEALTH AIDES:

  1. Oversee and evaluate the care provided by Home Health Aides.
  2. Document evaluations of Aides' performance.
  3. Engage in regular discussions with Home Health Aide supervisors.
  4. Maintain accurate records of patient care and changes in conditions.

JOB CONDITIONS:

  1. Must possess a valid driver's license and be able to travel to patient residences.
  2. Ability to access homes that may not be wheelchair accessible is essential.
  3. Physical activities include walking, sitting, and lifting patients as needed.
  4. Effective communication in English, both verbal and written, is required.

EQUIPMENT:

Utilize medical equipment such as thermometers, blood pressure cuffs, and glucometers.

COMPANY OVERVIEW:

Access to patient medical records and financial accounts is maintained under the supervision of the Director of Clinical Services.

QUALIFICATIONS:

  1. Current Washington State Registered Nurse license required.
  2. Preferred experience of at least one year in community or home health settings.
  3. Knowledge of home health care principles and required documentation.
  4. Strong organizational and time management skills are essential.
  5. Ability to communicate effectively with nursing staff and management.
  6. Must pass a criminal background check.
  7. Current CPR certification is mandatory.
