Job Description: As a Primary Care Specialist with UnitedHealth Group, you will have the opportunity to work with a diverse population of patients, providing high-quality care and support in the comfort of their own homes. You will be responsible for conducting home visits, performing physical exams, reviewing medical histories, and providing education on various health topics.
Responsibilities:
- Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings, including point of care testing (as appropriate)
- Identify diagnoses, formulate a list of current and past medical conditions, and communicate findings to patients and other healthcare professionals
- Educate members on various health topics, including disease process, medication, and compliance
Requirements: To be successful in this role, you will need an active NP license or ability to obtain by start date, as well as ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification. You must also have reliable transportation to complete home visit assessments.

Our salary is competitive and ranges from $88,000 to $173,200 annually, depending on experience and qualifications.
