Job Summary:

Lagniappe Rehab Home Care is seeking a skilled Occupational Therapist to provide high-quality care to patients in their homes. As an Occupational Therapist, you will work closely with patients, caregivers, and healthcare professionals to develop and implement personalized treatment plans that promote independence, mobility, and overall well-being.

Key Responsibilities:
  • Evaluate patients' physical and cognitive abilities to identify areas for improvement
  • Develop and implement individualized treatment plans to address patients' needs and goals
  • Provide hands-on therapy sessions to patients, including exercises, activities, and education
  • Collaborate with patients, caregivers, and healthcare professionals to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Graduate of an ACOTE-accredited school of Occupational Therapy
  • Valid National Board for Certification of Occupational Therapy (NBCOT) certification for Occupational Therapy
  • Valid state license (where appropriate)
  • Valid driver's license
Preferred Qualifications:
  • Experience working in home health or skilled nursing settings
  • Knowledge of Medicare and Medicaid regulations
  • Ability to work independently and as part of a team
