Texas Health Physicians Group is seeking a highly skilled and compassionate Palliative Care Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide comprehensive palliative care services to patients and their families dealing with serious illnesses.
Key Responsibilities:
- Provide counseling and instructions to patients in a home health and inpatient setting
- Collaborate with healthcare teams to develop and implement individualized care plans
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement strategies to improve patient outcomes and quality of life
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
Requirements:
- Graduate of an accredited Nurse Practitioner program
- NP Certification and Texas license required
- At least 1-2 years of Palliative Care experience
