We are seeking a compassionate and dedicated Hospice Care Specialist to join our team at AccentCare, Inc. As a Hospice Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals and their families in need of end-of-life care.

Key Responsibilities
  • Provide direct patient care and support to individuals in need of hospice care
  • Assist with daily living activities, such as bathing, dressing, and grooming
  • Monitor and report any changes in patient condition to the healthcare team
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Provide emotional support and companionship to patients and their families
Requirements
  • Certified Nursing Assistant (CNA) license in the state of Oregon
  • Valid CPR certification
  • Ability to travel to all business locations
  • Meet the regulations and requirements of the state(s) in which the program provides service
