Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team in a Primary Care setting. As a Psychiatric Mental Health Nurse Practitioner, you will provide comprehensive care to patients with mental health conditions, working collaboratively with other healthcare professionals to ensure seamless care.

Responsibilities
  • Provide primary care services to patients with mental health conditions, including assessment, diagnosis, and treatment planning.
  • Collaborate with other healthcare professionals to ensure comprehensive care, including consultation with psychiatrists, psychologists, and social workers.
  • Develop and implement individualized treatment plans, incorporating evidence-based practices and medication management as needed.
  • Conduct regular follow-up appointments to monitor patient progress and adjust treatment plans as necessary.
  • Participate in multidisciplinary team meetings to discuss patient care and share best practices.
Requirements
  • Master's degree in Nursing with a focus on Psychiatric Mental Health Nurse Practitioner (PMHNP) certification.
  • 2+ years of experience in a Primary Care or Behavioral Health setting, with a focus on mental health conditions.
  • Current DEA certification and ability to prescribe medications as needed.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound clinical decisions.
