Transgender Health Navigator

BAMSI is seeking a highly motivated and compassionate Transgender Health Navigator to join our team. As a key member of our Adult Services department, you will play a vital role in providing health navigation services to transgender individuals in need.
Key Responsibilities- Perform health navigation services for transgender individuals, including supported referrals to address their full spectrum of needs.
- Work with the Director of HIV Services to develop strategies for outreach, recruitment, and service delivery.
- Screen, interview, and counsel individuals who feel they are at risk of HIV/STI/HCV infection, providing harm reduction information and strategies.
- Collect specimens for HIV/STI/HCV antibody tests and send them to the State Laboratory as needed.
- Provide education on gender-affirming procedures, hormone treatment, and risk reduction strategies, as well as supported referrals and health care navigation for transgender individuals.
- Collect data and complete paperwork timely and accurately for statistical and monthly reports.
- Initiate and conduct follow-up activities to assess the impact of services provided.
- Develop, provide, and document appropriate referrals in focused areas for clients in need of services.
- Provide transportation to clients as needed.
- Enroll clients and bystanders in the Narcan Pilot Program.
- Facilitate group-level interventions and health communications using DPH/OHA-approved curriculum.
- Work with the Program Director and persons served to assess the effectiveness of the Prevention, Counseling, Screening, and Referral Programs.
- Coordinate efforts with other HIV/AIDS and transgender health programs.
- Promote the program through community outreach and information dissemination.
- Network with community groups and agencies as needed to advance prevention and education models.
- Attend meetings, conferences, and training to expand knowledge of HIV disease and related topics.
- Follow a flexible schedule that accommodates required evening and weekend hours.
- Complete all necessary paperwork and documentation in a timely and accurate fashion.
- Establish and maintain good rapport and harmonious relationships with staff, clients, and other program-related individuals.
- Assist with case management responsibilities as needed.
- Perform duties consistent with agency/program policies and procedures.
- Execute duties to reflect reasonable safety standards.
- Transgender is a Bona Fide Occupational Qualification for this position.
- BA/BS preferred; or combination of experience and High School Diploma or GED plus outreach/education.
- Knowledge of HIV/AIDS and Harm Reduction principles and practices.
- Knowledge of the Brockton area transgender community.
- Training in safe practices for blood draw preferred or will be trained.
- Good organizational and communication skills (oral and written) required.
- Demonstrated ability to take initiative and work in a team and individual setting.
- Approachable personality and open-mindedness required.
- Familiarity with the Brockton area required.
- Must have a valid driver's license and own means of transportation.
