Job SummaryWe are looking for a dedicated and experienced LPN/RN to join our team and provide skilled nursing care to patients with Spinal Muscle Atrophy. The ideal candidate will have a strong background in home health nursing and be able to work independently with minimal supervision.
Responsibilities- Provide skilled nursing care to patients with Spinal Muscle Atrophy in their homes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure comprehensive care
- Monitor patient progress and adjust care plans as needed
To be successful in this role, you will need:
- Current LPN/RN licensure in the state of PA
- Experience in home health nursing, preferably with patients with Spinal Muscle Atrophy
