**About the Role**

Firefly Children and Family Alliance is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support and guidance to families and youth in need.

Key Responsibilities

  • Family Progress - Monitoring and Coaching (50%)
    • Collaborate with families to set and achieve goals
    • Provide education and support to families
    • Assist families in accessing community resources
    • Attend court hearings and provide recommendations
    • Communicate with schools and other institutions
    • Respond to crisis situations and provide stabilization
  • Service Plans / Goal Development (20%)
    • Develop and update service plans for each family
    • Build relationships with community agencies
    • Work with other social welfare agencies to provide comprehensive support
  • Intake and Needs Assessment (15%)
    • Conduct thorough assessments of family needs
    • Visit families to gather information and complete assessments
    • Follow professional standards and guidelines
  • Reporting (15%)
    • Maintain accurate and timely records
    • Prepare reports for the Department of Child Services and Probation office

Requirements

  • Bachelor's degree in social work or related field
  • At least 21 years of age
  • Valid driver's license and insurance
  • Ability to travel between appointments
  • Knowledge of social service systems and confidentiality practices
  • Appreciation for cultural differences and ability to respond accordingly
