Role OverviewThe Licensed Practical Nurse (LPN) is responsible for planning and overseeing patient care tailored to individual needs. This includes conducting patient interviews, documenting medical histories, and assessing physical conditions. The LPN will monitor vital signs such as pulse, blood pressure, temperature, and respiration, while providing essential routine care.
Key Responsibilities- Deliver personalized 1:1 care for clients, adhering to their care plans while respecting their dignity and independence.
- Monitor and report any abnormal findings in clients' vital statistics to the case manager and physician.
- Administer medications, assist with feeding, and provide oxygen and ostomy care as outlined in the client's care plan.
- Collaborate with other healthcare professionals, including Physical, Occupational, and Speech Therapists, to follow established routines.
- Maintain strict adherence to infection control policies, reporting any potential communicable diseases or infections.
- Document all nursing actions accurately and communicate effectively with family members, case managers, and physicians regarding patient progress and changes in condition.
- Order necessary supplies and medications to ensure adequate availability at all times.
- Ensure all patient needs are met in accordance with physician instructions.
- Monitor incidents involving clients, ensuring timely and accurate completion of incident reports.
- Maintain all medical equipment and report any malfunctions for repair.
- Perform additional duties as assigned.
- Completion of post-secondary education required for licensure as a Licensed Practical Nurse.
- Understanding of individual service plans, client rights, and health needs.
- Current licensure as a Licensed Practical Nurse in good standing.
- CPR Certification.
- Strong interpersonal skills, including customer service, patience, empathy, and compassion.
- Thorough knowledge of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for various medical conditions.
- Willingness to travel between facilities as needed.

Salary Range
