Job Summary

We are seeking a highly skilled Wound Care Nurse to join our dedicated team at Paramount Community Living and Rehab. The successful candidate will play a critical role in assessing, planning, and implementing comprehensive wound care for our residents.

About the Role

This is a challenging and rewarding position that requires a deep understanding of wound care management, excellent clinical skills, and a commitment to providing exceptional patient care.

Duties and Responsibilities
  • Assess and plan individualized care for residents with wounds, including evaluation of wound type, stage, and underlying conditions.
  • Administer wound care treatments, including dressing changes, debridement, and application of topical agents.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and develop treatment plans.
  • Monitor and document progress of wound healing, changes in condition, and response to treatments.
Requirements
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid state nursing license.
  • Wound Care Certification (WCC) and Infection Preventionist (IP) certification required.
  • Minimum of 1 year of experience in wound care, preferably in a skilled nursing facility or similar healthcare setting.
