Key Responsibilities

Patient Care Management
* Engage in interdisciplinary patient care discussions to assess progress and set goals.

* Formulate individualized care plans that include objectives and home care instructions in collaboration with team members, patients, and families.

* Supervise and delegate specific treatments to support staff while maintaining accountability for patient care.

Documentation and Evaluation
* Identify and document patient learning needs in their records.

* Clearly communicate information tailored to the understanding level of patients and families.

* Implement treatment plans and assess patient responses, documenting outcomes effectively.

* Contribute to the development of discharge plans that address patient and family concerns.

* Adjust care plans based on ongoing assessments.

* Ensure compliance with departmental documentation standards.

Educational Contributions
* Serve as a resource for colleagues within the rehabilitation department, offering mentorship and educational opportunities.

* Collaborate with educational institutions to enhance professional growth.

Compliance and Safety
* Report safety incidents promptly and participate in safety training programs.

* Maintain awareness of safety standards and ensure compliance with regulations affecting patient care.

* Complete all mandatory training and documentation in a timely manner.

* Meet departmental productivity standards and respond promptly to new orders.

* Attend departmental meetings and engage with meeting content actively.

Qualifications

New Graduates Welcome

Education
Bachelor's degree - Required

Master's degree - Preferred

Licensure
Current Arizona state license - Required

BLS certification (American Heart Association) - Required upon hire.

Certification specific to the profession - Preferred

Experience
Clinical documentation experience - Required
