We are seeking a highly skilled Healthcare Navigator with Lifestyle Modification Focus to join our team at Buenaventura Affiliated Physicians IPA. This exciting opportunity will enable you to work closely with patients, healthcare providers, and community members to promote healthy lifestyles and provide education on nutrition and wellness.



About the Opportunity




  • Assist with utilization management and authorization processes to ensure patients receive necessary care and services.
  • Coordinate quality management activities with primary care physician offices to enhance patient outcomes and satisfaction.
  • Acquire and maintain credentials for physician contracts, ensuring seamless collaboration and communication among healthcare providers.
  • Provide patient care coordination, answering questions, and facilitating call backs to physician offices to ensure timely and effective care.
  • Collaborate with healthcare providers to develop and implement personalized care plans that promote healthy lifestyles and address specific patient needs.


About You




  • You have a strong background in healthcare, with previous experience as a medical assistant or licensed vocational nurse (LVN).
  • You possess literacy with managed care software and have a solid understanding of the managed care and referral authorization process.
  • You hold a bachelor's or master's degree in healthcare administration, with a focus on healthcare management or a related field.
  • You are an excellent communicator, able to effectively interact with patients, healthcare providers, and community members to promote healthy lifestyles and provide education on nutrition and wellness.
