Job Title: Peer Navigator, Street Medicine

At Healthcare in Action, we are committed to delivering innovative healthcare solutions to patients experiencing homelessness. Our Street Medicine approach involves providing medical care directly to individuals in need, using a medically equipped van to reach those in parking lots, encampments, and parks.

Job Summary:

We are seeking a highly motivated and compassionate Peer Navigator to join our team. As a Peer Navigator, you will work closely with patients to support their health and well-being, providing education, peer support, and navigation to local resources. Your goal will be to empower patients to manage their own health and make positive behavioral changes.

Key Responsibilities:
  • Build strong relationships with patients through personal experiences and effective communication.
  • Coach patients to develop strategies that minimize health risks and promote positive behaviors.
  • Connect patients to local services and resources, ensuring they receive the support they need.
  • Provide advocacy and support to patients, delivering medications and supplies as needed.
  • Coordinate medical and social services appointments, ensuring patients receive comprehensive care.
  • Introduce tools and resources to promote self-management and self-efficacy.
  • Maintain accurate documentation and records, including patient touch points and encounter notes.
Requirements:
  • GED or high school diploma, plus 3+ years of experience working with patients experiencing homelessness.
  • Bachelor's degree preferred, with 1-2 years of experience in a similar field.
  • Direct experience, lived experience, or training working with patients experiencing homelessness and mental health/substance use conditions.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Reliable transportation and a valid driver's license.
