Job Summary:

The primary responsibility of this role is to assess the healthcare needs of individuals and develop effective care plans to support their personal goals and medical needs in the community setting. You will work closely with interdisciplinary teams to provide nursing services, including reviewing and completing level of care assessments, writing assessments, updates, technical assistance, and consultation.

Key Responsibilities:
  • Conduct comprehensive assessments of individuals' healthcare needs based on data collected through history, observation, or psychosocial examination, and analysis of diagnostic data.
  • Perform complete physical and psychosocial/Nursing assessments on individuals using all available sources of information, including individual and family medical records.
  • Develop and implement nursing care plans that support individuals' personal goals and medical needs.
  • Provide health education instruction to consumers on various health issues, such as treatments, self-care, and surgical procedures.
  • Assess individuals' health problems and needs, develop and implement nursing care plans, and maintain medical records.
  • Advise clients on health maintenance and disease prevention, and provide case management.
  • Communicate with agency personnel, service providers, support coordinators, and other interested parties to coordinate efforts and resolve operational problems.
Requirements:

To be successful in this role, you must have a current nursing license in the State of Georgia and be able to practice within the state. You will also be required to undergo evaluations, vaccinations, and professional licensure and certifications as necessary.
