Join our dynamic team at Sea Mar Community Health Centers as a Community Health Navigator. In this critical role, you will serve as a primary liaison between clients and our clinic, focusing on alleviating barriers to service and helping patients establish us as their medical home.

The estimated salary for this position is $20.00 per hour, based on industry standards and location. As a key member of our Managed Care Department, you will be responsible for assisting with public health coverage program applications, ensuring patients have selected a primary care provider, assisting in scheduling appointments, and identifying other healthcare services available.

You will also perform outreach activities, participate in corporate marketing campaigns, and distribute informational/promotional materials approved by the Managed Care Department. Additionally, you will maintain activity records, submit progress reports as directed by the Managed Care Department, and strive to increase customer healthcare service utilization through education on services covered and customer responsibilities.

To succeed in this position, you must have excellent communication and interpersonal skills, be able to interpret instructions furnished in written or oral form, and possess a high school diploma or equivalent. A Bachelor's degree in Social Services or a related field is preferred. Bilingual in English/Spanish is also highly desirable.
