Job Opportunity

We are seeking a skilled Physician to join our team at Mountain View PACE. As a Geriatric Medicine Specialist, you will play a vital role in providing high-quality care to our elderly patients.

Key Responsibilities:

  • Perform interval history and physicals on established participants
  • Evaluate and treat Participants with episodic illnesses
  • Refers Participants to and oversees care by medical specialists as indicated
  • Maintain required privileges with contracted and ad hoc facilities to maintain continuity of care
  • Responsible for coordinating discharge planning with the team
  • Manage all care of Participants in the nursing home
  • Perform telephone visits with nursing home staff as required
  • Admit nursing home Participants to the hospital when necessary
  • Actively participate in family conferences
  • Participate in team meetings, committees, and work groups as assigned
  • Communicate participant changes to team members
  • Participate in the development of the IDT Plan of Care
  • Participate in the coordination of a 24-hour care delivery
  • Complete all documentation in the medical record as required
  • Work collaboratively, including prescriptive authority, with nurse practitioners

Requirements:

  • Physician holding MD degree and board certified in Internal Medicine
  • Prefer Board Certification in Geriatric Medicine
  • Prefer 2 – 5 years' experience in Geriatric Medicine
  • The ability to lead and work in an interdisciplinary setting
  • Ability to deliver service to physicians, patients, and fellow employees with compassion in a responsive, courteous, and concerned manner following PACE and Pre-PACE policies and procedures
  • Ability to manage changing priorities in response to program needs
  • Must meet a standardized set of CMS approved competencies before working independently
  • Must be legally authorized to practice in the state of Kentucky. Licensed physician in the State of Kentucky
  • Experience or advanced degree in geriatrics is required. Current state license, DEA registration and staff privileges at Mountain View PACE and contracted hospitals are required
  • Must have 1 year of experience with a frail or elderly population
