Responsibilities:
- Assess patient knowledge of diet and kidney disease, providing education and counseling to promote healthy eating habits and disease management.
- Develop and implement individualized nutrition plans, working with patients, families, and caregivers to achieve optimal nutritional status.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans, ensuring that patients receive the best possible care.
- Monitor patient progress, adjusting nutrition plans as needed to achieve optimal outcomes.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in renal nutrition.
- Stay up-to-date with the latest research and developments in renal nutrition, applying this knowledge to improve patient care.
Requirements:
- Registered Dietitian (RD) or Board Certified Specialist in Renal Nutrition (BCSRN) certification.
- Minimum 1 year of experience in clinical nutrition, preferably in a renal setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Basic computer skills and proficiency in electronic medical records.
