Job Summary

The Advanced Practice Provider (APP) practices in a clinic as part of a collaborative team that may include, but is not limited to, Family Practice and/or Internal Medicine Physicians, RN Case Managers, and clinic staff.

The purpose of this collaborative team is to provide a comprehensive, coordinated model of care.

Key Responsibilities:

  • Manage patient populations in conjunction with primary care physicians and specialists.
  • Provide chronic and acute care to patients in outpatient and inpatient settings.
  • Act as an extension of the primary care physician to provide high-quality patient care.
  • Diagnose and treat acute and chronic conditions, including diabetes, hypertension, and kidney disease.
  • Order and interpret diagnostic lab work, x-rays, and EKGs.
  • Prescribe medications and other treatment modalities as needed.
  • Counsel patients on disease conditions and treatment options.
  • Work collaboratively with a comprehensive team to provide quality, evidence-based care.

Requirements:

  • Graduate credit for PA/NP Program
  • Current licensure through the Georgia Board
  • Current prescriptive authority from the Georgia Board
