The responsibilities include:

  1. Coordinating various self-management support and disease registry initiatives for the specified population.
  2. Utilizing data analytics to manage the health outcomes of the identified patient group.
  3. Engaging closely with physicians, administrative personnel, and staff across all facilities to review medical records, provide counsel on quality-related incidents, and present data to support the overall quality of care.

EDUCATIONAL REQUIREMENTS

  1. Education: A Registered Nurse (RN) license is mandatory; a Bachelor of Science in Nursing (BSN) is preferred.
  2. Experience: A minimum of three (3) years of experience in a hospital setting is required.
  3. Licensure: A current RN license from the state of Washington is essential.
  4. Additional Skills: Bilingual capabilities, particularly in English and Spanish, are preferred, along with the ability to engage with a culturally diverse patient population.

Salary Range: $54.03 per hour
