The responsibilities include:
- Coordinating various self-management support and disease registry initiatives for the specified population.
- Utilizing data analytics to manage the health outcomes of the identified patient group.
- Engaging closely with physicians, administrative personnel, and staff across all facilities to review medical records, provide counsel on quality-related incidents, and present data to support the overall quality of care.
A key attribute for success in this position is exceptional interpersonal customer service, characterized by a positive, warm, and respectful demeanor towards patients, families, colleagues, and other stakeholders.
EDUCATIONAL REQUIREMENTS
- Education: A Registered Nurse (RN) license is mandatory; a Bachelor of Science in Nursing (BSN) is preferred.
- Experience: A minimum of three (3) years of experience in a hospital setting is required.
- Licensure: A current RN license from the state of Washington is essential.
- Additional Skills: Bilingual capabilities, particularly in English and Spanish, are preferred, along with the ability to engage with a culturally diverse patient population.
Salary Range: $54.03 per hour
