About the Role

Tufts Medicine Care at Home is seeking a skilled Hospice RN to provide high-quality nursing care to patients and their families in various settings. As a member of our team, you will be responsible for delivering compassionate and expert care, adhering to our organization's core values.

Key Responsibilities
  • Provide hospice nursing care to patients and families, utilizing current principles of palliative care and symptom management to achieve optimal outcomes.
  • Utilize the nursing process and adhere to current standards of nursing practice in providing hospice care.
  • Remain competent on technical nursing skills, including updates to care plans, medication profiles, and HHA treatment plans.
  • Complete all nursing documentation according to procedures and nursing standards of practice.
  • Involves Team Leader and Medical Director in situations as needed.
  • Assists in the orientation of new staff and mentors LPNs and nursing students as requested.
  • Pursues standards of excellence in performance as a Hospice RN.
  • Works with Team Leader and Clinical Manager to identify opportunities for improvement in performance and addresses them.
