Job Summary:

Tufts Medicine Care at Home is seeking a skilled Hospice Nurse to provide high-quality care to patients in various settings, including private homes, assisted living facilities, skilled nursing facilities, and hospice inpatient facilities.

Key Responsibilities:

  • Deliver quality hospice nursing care to patients and families, utilizing current principles of palliative care and symptom management to produce hospice outcomes.
  • Utilize the nursing process and current standards of nursing practice in providing hospice care.
  • Remain competent on technical nursing skills, including IVs, G-tubes, etc.
  • Understand family dynamics and work effectively within various types of family systems.
  • Utilize resources appropriately, demonstrate the ability to act as a patient and family advocate, and provide information to families about all available resources.
  • Accurately address need for additional services, communicate patient reports and status changes concisely and effectively, and assess patient for need for Fall Precautions.
  • Update Care Plan, Medication Profile, and HHA Treatment Plan routinely and as patient condition changes.
  • Complete all nursing documentation per procedures and nursing standard of practice, including assessments, interventions, responses to interventions, communications, verbal orders, etc.
  • Involves Team Leader and Medical Director in situations appropriately.
  • Understand how to maintain professional boundaries with patients and families.
  • Assist in orientation of new staff and mentor LPNs and nursing students as requested.
  • Pursue standards of excellence in performance as a Hospice Nurse.
  • Work with Team Leader and Clinical Manager to identify opportunities for improvement in performance and work to address them.
