We are seeking a highly organized and detail-oriented Care Coordinator Assistant to provide strong clerical support to our unit-based care managers. The successful candidate will handle all clerical responsibilities, ensuring seamless care coordination and administrative tasks.
Key Responsibilities- Administrative Support: Provide clerical support to care managers, including issuing notifications, scheduling appointments, and assisting with transportation arrangements.
- Discharge Coordination: Assist care managers with discharging patients, utilizing a discharge checklist and working with Teletracking milestones.
- Receptionist Duties: Perform receptionist duties, including greeting and directing employees and visitors, responding to telephone inquiries, and creating required forms for documentation.
- Filing and Tracking: Create a filing and tracking system for the storage and retrieval of archived documents and maintain statistics for submission to the Director of Care Management.
- Communication: Communicate effectively with staff, management, and external constituencies, demonstrating excellent telephone manners and good communication skills.
- Education: High School Graduate Required
- Experience: 3 years' experience in a Hospital Care Management Department highly preferred
- Knowledge: Knowledge of Insurance and Medicare regulations, discharge planning process and regulations, and medical terminology
- Core Competencies: Ability to speak, write, understand, and communicate in English language, perform multiple duties in a high-volume area, and demonstrate analytical skills, accuracy, and attention to detail.
The successful candidate will work in a fast-paced environment, exposed to chemicals necessary to perform required tasks. Must be physically able to stand for extended periods and perform word processing/computer work and use of transcription equipment.
Primary Population ServedThe Care Coordinator Assistant will work with various patient populations, including neonatal, infant, pediatric, adolescent, adult, and geriatric patients, as well as patients with exceptional communication needs, developmental delays, and cultural needs.
