Responsibilities
The Physical Therapist is responsible for delivering physical therapy services, communicating patient progress with other healthcare professionals, and overseeing the non-professional home health aide staff in providing personal care to patients.

As a Physical Therapist, your responsibilities will include:
- Delivering Physical Therapist services, including assessment, evaluation, procedures, and identifying mutually agreed-upon goals, as well as teaching and training activities as outlined in the patient Plan of Care.
- Continuously updating care plans as changes occur and during recertification.
- Submitting completed Physical Therapist visit notes as required by the Clinical Supervisor.
- Conducting all OASIS time point assessments and managing the discharge planning process in accordance with Medicare criteria, policies, and procedures.
- Participating in staff meetings, team conferences, and educational in-services as necessary.
- Performing additional duties as assigned by the Administrator.
- Ensuring the application of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care.

Qualifications
- A current, unencumbered State Physical Therapy License in the state of practice.
- A valid driver's license, a good driving record, proof of insurance, and a reliable mode of transportation.
- A minimum of one year of experience as a home care professional Physical Therapist, with competence in performing comprehensive assessments and OASIS-C preferred.
- The ability to exercise sound professional clinical judgment.
- Proficiency in assessing and documenting patient needs and formulating individualized care plans.
- Strong clinical skills and proficiency in Microsoft Office.
- Capability to drive for 2-4 hours daily.
