Job Title: Cardiovascular Rehabilitation Nurse Coordinator
Description:
We are seeking a skilled Registered Nurse to lead our Cardiac Rehabilitation program. The successful candidate will be responsible for coordinating and monitoring all clinical aspects of the program, ensuring participant safety, and facilitating effective communication among team members.
Responsibilities:
- Coordinate the potential participant through the screening and intake process
- Determine the eligibility of potential participants with active oversight from the Medical Director
- Responsible for completing electronic health records and data collection throughout the 72-hour program and at one year
- Perform comprehensive health assessments at baseline and before each intervention session to ensure safe participation
- Access specialists and resources as needed during the screening and intake process
- Complete reimbursement procedures as outlined by payers
- Attain electronic health record super user status and monitor ongoing documentation of each participant's health status
- Coordinate the development, implementation, and evaluation of interdisciplinary treatment plans for participants
- Perform regular health assessments and update treatment plans as needed
Requirements:
- Diploma, Associate, Bachelor, Masters, or PhD degree in Nursing
- Active Registered Nurse license in good standing from the state board of nursing
- Minimum of two years of clinical experience in adult cardiovascular nursing and/or cardiac rehabilitation
- Formal education/training in cardiac rehabilitation and/or exercise physiology preferred
- Current certification in Advanced Cardiac Life Support (ACLS)
- Computer literacy
- Excellent organizational and time management skills
- Excellent interpersonal communication skills with compassion, empathy, and sensitivity to participant needs
- Excellent public speaking skills and knowledge of adult education and coaching principles
Salary Range: $29.00 - $32.50 per hour
