- Evaluate residents' conditions and report changes to physicians in a timely manner.
- Utilize Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR) systems to ensure accurate and efficient care.
- Communicate effectively with team members, residents, and families to build strong relationships and ensure seamless care transitions.
- Develop and implement care plans that meet the unique needs of each resident.
- Licensed Practical Nurse (LPN) in the state with Assisted Living/Memory Care experience preferred.
- Prior supervisory experience required.
- Punctuality and professionalism are essential for success in this role.
- Passion for working with the elderly and a commitment to delivering exceptional care.
