Program Insight:

The United Healthcare at Home initiative is a comprehensive care delivery model that facilitates clinical care for members in their residences. This program emphasizes personalized interventions tailored to individual needs, in collaboration with an Interdisciplinary Care Team, which includes clinicians, primary care providers, and other healthcare professionals.

Core Work Schedule: Monday – Friday, 8:00am – 5:00pm

Key Responsibilities:

  • Evaluate, strategize, and execute individualized care management interventions for each patient, aiming for the most suitable and least restrictive care level.
  • Conduct timely outreach and in-person visits for members newly eligible for services, completing necessary assessments in the electronic medical record (EMR).
  • Identify and initiate referrals for healthcare and community-based services, including financial, psychosocial, and state support services.
  • Develop and implement care plans throughout the care continuum, serving as a primary point of contact.
  • Communicate essential health-related information to all stakeholders to ensure quality, coordinated care.
  • Advocate for patients and families to ensure their needs and preferences are represented within the healthcare team.
  • Identify appropriate interventions and resources to address gaps in care based on individual needs.
  • Document care plans in relevant electronic health record systems and maintain accurate data entry.
  • Engage consumers by building rapport, demonstrating empathy, and fostering trust.
  • Collaborate with primary providers and multidisciplinary teams to align care goals and enhance coordination.
  • Provide ongoing support for advanced care planning.
  • Reassess care plans at appropriate intervals based on objectives or significant changes in condition.
  • Understand and operate within legal and regulatory frameworks.
  • Utilize evidence-based guidelines to inform care practices.
  • Make and receive calls to assess members' health statuses and identify treatment barriers.
  • Educate patients to support self-management of their health.
  • Facilitate referrals to external resources as needed.
  • Ensure a complete continuum of quality care through effective communication with members.
  • Support members with education on conditions, medication reviews, and connections to resources.

This role operates in a high-volume, customer-focused environment, requiring efficiency and thoroughness in member interactions. Strong computer skills and adaptability to change are essential.

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license.
  • 1+ years of experience in a community health setting.
  • 1+ years of experience in community-based care management for long-term care or home health.
  • Demonstrated competency in managing intensive case management services.
  • Must reside within a specified distance of the service area.

Preferred Qualifications:

  • Certified Case Manager (CCM) certification.
  • Experience managing populations with complex medical or behavioral needs.
  • Experience with home care services and community-based settings.
  • Experience in case management and related areas.
  • Field-based work experience.

Additional Considerations:

  • Knowledge of community organizations providing resources for members.
  • Understanding of Medicaid LTSS and behavioral health services.
  • Ability to travel within the assigned region for member visits.
  • Proficient in computer skills, including EHR documentation.
