The Manager of Case Management Services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department.
Key Responsibilities- **Serves as a Clinical Resource**: Functions as a role model and clinical resource for staff, ensuring they are competent in the use of InterQual to progress the plan of care.
- **Supervises and Develops Staff**: Supervises and facilitates development of staff, overseeing the day-to-day tasks/activities in the department.
- **Collaborates with Interdisciplinary Team**: Works collaboratively and maintains active communication with physicians, nurses, and other members of the interdisciplinary care team to effect timely, appropriate patient management.
- **Monitors Quality and Documentation**: Monitors the quality of CM interactions with patients, families, physicians, and the multidisciplinary team by regularly shadowing staff to ensure clear understanding and adherence to department processes.
- **Communicates Effectively**: Communicates effectively with patients, family, medical staff, care team members, students, supervisors, and outside contacts.
- **Education**: Associate Degree in Nursing or Nursing Diploma.
- **Experience**: 3+ years experience in clinical nursing.
- **Credentials**: Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
