About the Job:

This is an excellent opportunity to work with a leading healthcare provider in a dynamic and fast-paced environment. As a Patient Care Coordinator, you will play a vital role in ensuring seamless patient experiences through virtual consultations.

Responsibilities:
  1. You will receive inbound calls and make outbound calls to patients, caregivers, pharmacies, and specialist offices.
  2. Perform comprehensive telephonic patient assessments, following established clinical protocols and guidelines.
  3. Complete documentation of telephonic conversations with patients, caregivers, pharmacies, and specialist offices.
  4. Follow up on failed faxes to pharmacies and access the Language Line as needed.

Requirements:

  • A minimum of two years of nursing experience, preferably with Patient First.
  • Strong oral, written, and interpersonal communication skills, with ability to work independently.
  • Familiarity with computer systems and automated call distributor telecommunication systems.
