Job Summary

The Hospice and Palliative Care Physician will work collaboratively with the hospice team to provide high-quality, patient-centered medical care with a focus on symptom management and end-of-life care to hospice patients under the direction of the Hospice Medical Director.

Key Responsibilities
  • Collaborate with the interdisciplinary team to develop, implement, and revise care plans that reflect the patient's goals, values, and physical, emotional, and spiritual needs.
  • Certify in writing that each individual considered for admission to hospice or continuing hospice care is terminally ill, in accordance with applicable laws, regulations, and hospice policies.
  • Engage in independent decision-making about patient healthcare concerns and provide hospice services as needed, including managing urgent medical issues and providing education, counseling, and guidance to patients, families, and caregivers.
  • Attend interdisciplinary team meetings, provide medical expertise, and collaborate with the team to ensure compliance with medical standards of care for hospice patients.
  • Provide consultation, support, and education to the hospice team, attending physicians, and other colleagues regarding medical eligibility, symptom management, and other issues related to hospice care.
Requirements
  • MD or DO degree
  • Experience in hospice and palliative care
  • Strong clinical skills and ability to work independently
  • Excellent communication and collaboration skills
