Job SummaryWe are seeking a highly skilled Physician Assistant to join our Hospice and Palliative Medicine team. As a Physician Assistant, you will work closely with our interdisciplinary team to provide comprehensive care to patients with serious illnesses.
Key Responsibilities- Assess and manage patients with complex medical needs, including pain and symptom management
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Provide education and support to patients and families on disease management, treatment options, and end-of-life care
- Collaborate with healthcare teams to coordinate care and ensure seamless transitions
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
- Master's degree in Physician Assistant Studies or equivalent
- Current certification as a Physician Assistant
- Experience in Hospice and Palliative Medicine or a related field
- Strong communication and interpersonal skills
- Ability to work in a fast-paced, dynamic environment
