At HOSPICE of SURRY COUNTY, we're seeking a dedicated Registered Nurse to join our team as a Hospice Nurse - Patient Care Specialist. As a key member of our interdisciplinary team, you'll play a vital role in providing exceptional care to patients and their families.
Key Responsibilities:- Conduct thorough nursing evaluations and develop personalized care plans in collaboration with the physician and interdisciplinary team.
- Continuously assess and re-evaluate patient needs, updating the Plan of Care as needed to ensure the highest quality of life.
- Maintain accurate, up-to-date documentation using our Electronic Medical Record (EMR) system and prepare timely clinical progress notes.
- Participate in interdisciplinary team meetings, contributing valuable insights to enhance patient care.
- Stay current with the latest in hospice care by embracing best practices, research, and ongoing professional development.
- A balanced schedule with 4 ten-hour shifts per week and every 3rd weekend rotation, with 8 hour shifts also available.
